Минтруд России разработал форму для сбора сведений о заработной плате работников бюджетной сферы, а также инструкцию по ее составлению (письмо Минтруда России от 15 марта 2021 г. № 14-5/10/П-1949). Направить отчет нужно до 9 апреля 2021 года в ПФР по утвержденным Фондом форматам в форме электронного документа по существующим телекоммуникационным каналам связи, предусмотренным для представления сведений для ведения персонифицированного учета.
Форму заполняют государственные и муниципальные учреждения. Состоит она из трех разделов: в первом указываются сведения о заработной плате работников, во втором сведения о фонде заработной платы, в третьем – сведения о заработной плате руководителей, заместителей руководителей и главного бухгалтера учреждения.
Первый раздел заполняется для каждого физлица (номера СНИЛС), работавшего по трудовому договору в отчетном месяце, независимо от того, работает ли работник на момент сдачи отчета. При этом указывается, что гарантируется конфиденциальность. Данные должны быть предоставлены по каждой занимаемой сотрудником должности отдельно, при этом номер СНИЛС повторяется в каждой строке. Например, если физическое лицо занимает 1 ставку по одной должности, а 0,5 ставки по аналогичной или иной должности, то записей должно быть две.

Также в этом разделе указывается код, за который выгружается отчет, месяц, за который предоставляются сведения о начисленной зарплате, ИНН и КПП организации, а также ОКФС и код типа организации. Далее идет раздел с данными о сотруднике, в который заносятся Ф. И. О. и СНИЛС работника, его общий трудовой стаж, должность, код категории персонала и условия занятости, ставка, количество отработанных часов по графику и фактически, а также произведенные начисления, доплаты и выплаты за месяц.
Во втором разделе указывается объем расходов фонда зарплаты из федерального, регионального и местного бюджетов, объем расходов на счет средств ОМС и объем расходов из иных источников. В третьем разделе нужно заполнить информацию о среднемесячной начисленной зарплате руководителя организации, его заместителей, главного бухгалтера и всех остальных работников организации.
