Правительство РФ разработало формы заявления о назначении ежемесячного пособия женщине, вставшей на учет в медицинской организации в ранние сроки беременности, а также заявления о назначении ежемесячного пособия на ребенка в возрасте от 8 до 17 лет, а также порядок их заполнения (постановление Правительства РФ от 28 июня 2021 года № 1037). Кроме того, опубликован перечень документов (копий документов, сведений), необходимых для назначения вышеуказанных ежемесячных пособий. С текстом документа можно ознакомиться на официальном сайте Правительства РФ.
Согласно правилам назначения и выплаты пособия, они предусмотрены для:
беременных женщин, вставших на учет в медорганизации в ранние сроки беременности (до 12 недель), если срок их беременности составляет шесть и более недель, и размер среднедушевого дохода их семей не превышает величину прожиточного минимума на душу населения в субъекте РФ по месту их жительства (пребывания) или фактического проживания; единственного родителя ребенка в возрасте от 8 до 17 лет или родителя (иной законный представитель) такого ребенка, в отношении которого родителю (иному законному представителю) предусмотрена уплата алиментов, при этом размер среднедушевого дохода такой семьи не превышает величину прожиточного минимума на душу населения в субъекте РФ по месту жительства (пребывания) или фактического проживания заявителя.

Заявления о назначении пособий подаются в территориальные органы ПФР. Представить документы можно в электронном виде на портале госуслуг или лично по месту жительства (пребывания) или фактического проживания заявителя. Если заявление подано электронно, то заявитель в течение 10 рабочих дней со дня регистрации заявления должен представить в отделение ПФР необходимые документы. В случае личного представления заявления документы можно подать сразу же, но если представлен не полный комплект, заявителю также дается 10 рабочих дней на представление необходимых документов (сведений), предусмотренных перечнем, в полном объеме.
Решение о назначении или об отказе в назначении пособия принимается отделением ПФР в течение 10 рабочих дней со дня регистрации заявления. Выплата пособий осуществляется с 1 по 25 число месяца, следующего за месяцем назначения выплат.
